The PASSPORT Medicaid Waiver Program provides Ohioans in-home services designed to help seniors who are at risk of nursing facility placement to live safely at home.
Participants have a custom service plan developed to meet their needs at home or at the home of a family member or friend. Services complement the care and assistance provided by family and friends and can be adjusted as needs change.
